My mother is a patient of Dr. Michael Moua at your Southgate location. I am writing to express my frustration with your office. My mother suffered from a stroke on April 18, 2021 and was released from the SNF on May 05, 2021 to continue her rehabilitation at home. I, her son, requested leave from work to care for my mother while the SNF works on getting home health services set up for her. I brought in my work documents for Dr. Moua to sign on May 06, 2021. It has been 7 days since I requested your assistance. I have made several attempts to call your office and no one is answering the phone. I sincerely request for your assistance so I can care for my mother while I try to save my job as well. Please respond as soon as possible. I’d greatly appreciate it.
